After this form is submitted, stop by the CPC carport on a Monday and Thursday between 9 am & 12 pm to prepay and secure your pet's appointment. SPAY & NEUTER APPOINTMENTS * Dates subject to availability. We accept cash (exact amount) and money orders. Cost: includes 5 services: $100 for cats and $110 for dogs*(Dogs must weigh 50 # or less less to qualify). * If you believe that your dog weighs 50 pounds or more, you must bring your pet to the CPC to be weighted prior to scheduling an appointment.** For all RC residents regardless of income. Individual forms are necessary for each animal. *** If you miss your appointment, you will not receive a refund as we will be charged the full amount of your pet/s" surgical appointment/s.
